Job Description:
The RN will conduct initial and ongoing assessments, develop and adjust care plans, administer medications, provide patient education, and coordinate care as needed. Experience in Home Health & Hospice is required, and candidates must possess a valid Driver's License and Auto Insurance.
Responsibilities:
- Initial assessment of client and family to determine home health needs and complete physical assessment & history of current & previous illness.
- Regularly re-evaluates client nursing needs and uses health assessment data to determine nursing diagnosis.
- Initiates plan of care and makes necessary revisions as client status & needs change.
- Administers medications and treatments as prescribed by the physician.
- Instructs, supervises and evaluates home health aide care provided.
Requirements:
- BLS Certification
- Home Health & Hospice experience required
- Must have current Driver's License & Auto Insurance
