Job Description:

The RN will conduct initial and ongoing assessments, develop and adjust care plans, administer medications, provide patient education, and coordinate care as needed. Experience in Home Health & Hospice is required, and candidates must possess a valid Driver's License and Auto Insurance.

Responsibilities:

  • Initial assessment of client and family to determine home health needs and complete physical assessment & history of current & previous illness.
  • Regularly re-evaluates client nursing needs and uses health assessment data to determine nursing diagnosis.
  • Initiates plan of care and makes necessary revisions as client status & needs change.
  • Administers medications and treatments as prescribed by the physician.
  • Instructs, supervises and evaluates home health aide care provided.

Requirements:

  • BLS Certification
  • Home Health & Hospice experience required
  • Must have current Driver's License & Auto Insurance
