Requirements
- Activity logs
- If they aren t board certified (in any board- typically new grads) they will need their life support certs, ACLS, BLS, PALS (pediatrics)
- They do not need the sedation exam if they are Emergency Medicine Board Certified
- References must come from a department Chair/Medical Director at all current facilities and usually they will like to see any from affiliations that ended within a year
- Active MD license
- Active DEA/CSR
