We are seeking a qualified and experienced advanced care nurse practitioner who is looking to work in a dynamic post-acute setting. This role is ideal for someone who enjoys setting their own schedule, takes full ownership of their work, is a team player, and is an excellent communicator and relationship builder. Most importantly, we are looking for someone truly passionate about the care they provide.
To begin with, this position will involve rounding in a large skilled nursing facility with a patient volume of around 100. With the right personality and motivation, this role has the potential to grow significantly. As the patient volumes increase, an advanced care provider will be available to assist with follow-ups and discharges. The earning potential in this role is uncapped.
