• Other RadOncs: Present during coverage
• APP: 1 during coverage
• Patient Coverage: Outpatient
• Average Patients Per Day: 20-30 on treatment
• Patient Population: Adult
• EMR: Epic

Required Procedures/Duties:

• Conventional Radiotherapy
• External Beam Radiation - IMRT
• Stereotactic Body Radiation Therapy (SBRT)

Submission Requirements:

To expedite the credentialing process, the following documents must be available at the time of acceptance:

• Board Certification: Required
• Ohio State License: Preferred; providers willing to obtain a license are accepted
• BLS/ACLS Certification: Required
• Availability: Full-time availability only
• Flu Vaccine: Required at the time of credentialing
• COVID Vaccine: Preferred
