The VA clinic in East Portland/Fairview seeks a full-time board-certified or eligible Family Practice or Internal Medicine physician to serve as Primary Care Clinical Practice Manager within a PACT interdisciplinary outpatient team, caring for a panel of ~900 veterans. The role combines independent primary care practice with leadership and administrative responsibilities, promoting staff education, interprofessional training, and quality patient management for acute and chronic conditions. Full-time, Monday–Friday schedule, federal benefits, potential OHSU academic appointment, and requirements include US citizenship, unrestricted US medical license, and board certification.
The East Portland/Fairview, Oregon Veterans Administration Community-Based Outpatient Clinic is seeking a full-time Board-Certified/Board-Eligible Family Practice or Internal Medicine physician to join our out-patient clinic in Fairview, Oregon. This position will serve as the Primary Care Clinical Practice Manager Physician. As part of the Portland VA Medical Center, you will provide services within an integrated network of care facilities and a distributed system of outpatient clinics and hospitals across Oregon. The primary care provider will function as a highly skilled, independent care provider in patient management, working within an interdisciplinary care team with access to state-of-the-art medicine and the most advanced medical technologies available.
Candidates must be board certified in Family Practice or Internal Medicine and demonstrate leadership and administrative capabilities. The Health Care Clinic operates under the PACT Model (Patient Aligned Care Team). Along with their team, consisting of a PCP, RN, LPN, and a Medical Support Assistant, the PCP will care for a panel of approximately 900 patients. The PACT team provides comprehensive care for our Veteran population, covering diagnoses, care, and treatment of both acute and chronic conditions. Common conditions include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. The position is Monday through Friday with no weekends or holidays.

Requirements:
- Must be a U.S. Citizen
- Current, full, and unrestricted license to practice medicine or surgery in the U.S.
