A Family Practice - Geriatrics Physician provides comprehensive primary care to geriatric patients, coordinating specialist referrals and various healthcare services. This role involves active participation in care teams, medical documentation, and adherence to clinical protocols to ensure high-quality patient outcomes. The physician is responsible for fostering a collaborative environment while maintaining strong communication and patient service skills in a value-based care model.
Job Title: Primary Care Physician (PCP)
Location: Mississippi City, MS
Salary: $250k-330k
Key Responsibilities:
- Provide high-quality care to patients in accordance with established standards.
- Coordinate patient services, including referrals to specialists, hospital and SNF coordination, durable medical equipment, and home health care.
- Actively participate in daily care team huddles, contributing medical expertise.
- Foster a cooperative and professional practice environment alongside the Lead Physician and Center Administrator.
- Complete medical record documentation daily, collaborating with quality-based coders to ensure comprehensive documentation.
- Adhere to clinical leadership policies and protocols.
- Regularly meet with the Lead Physician to review quality of care, outcome data, and medical records.
- Focus on direct patient care, including patient interactions and related administrative duties (charting, meetings, etc.).
Qualifications:
Required:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license or willingness to obtain a license in the state of practice; eligibility and willingness to obtain licenses in other regional states as needed.
- Board Certification or eligibility (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid provider numbers.
- Strong interpersonal and communication skills with colleagues and patients.
- Commitment to the team-based care model.
- Willingness to learn and adapt to a value-based care practice.
- Excellent patient/customer service skills.
- Basic computer skills, including email and EMR.
- This role involves direct patient interaction; TB screening is required if selected.
Preferred:
- Knowledge of HEDIS/STARS, CAHPS, and HOS.
- Experience working in medically underserved or culturally diverse communities.
- Familiarity with electronic health records.
- Experience in Medicare Advantage Global Risk Models.
