Responsibilities:

• Provide compassionate, evidence-based care for older adults with acute and chronic conditions, using sound medical judgment and consulting specialists as needed.
• Ensure compliance with all clinical, regulatory, and ethical standards, including HIPAA, CLIA, and accrediting bodies.
• Engage in all aspects of patient care: assessment, diagnosis, treatment, education, and care coordination, emphasizing prevention and outcomes.
• Maintain accurate, timely EMR documentation within 48 hours of each visit.
• Collaborate with hospitalists during inpatient stays and remain involved in patient care.
• Supervise Nurse Practitioners/Physician Assistants when required and ensure patients are seen by a physician at least every other visit.
• Demonstrate professionalism, quality, and a commitment to 5-star service aligned with the Group s mission.

Curriculum Vitae

Copy of AL Medical License

DEA

BLS, ACLS

Working knowledge with Google Suite and Microsoft Office for real-time collaboration

At least 3 years of outpatient experience
