Minimum 2 years of nursing experience
Strong assessment skills and ability to respond to medical or psychiatric changes promptly

Strong communication, organization, and documentation skills
Flexibility to work a 35-hour full-time schedule and participate in on-call phone consultation

Position OverviewThis full-time RN will provide integrated nursing services across Crisis Respite/Transitional Programs (CRTP), Intensive Case Management (ICM), and Critical Time Intervention (CTI). The role focuses on stabilizing individuals transitioning from inpatient treatment, supporting medical and behavioral needs, and ensuring continuity of care in the community.

Participate in admission screenings and intake for Crisis Respite and Transitional Respite programs.
Complete initial and ongoing nursing assessments.
Oversee self-administration of medication and monitor therapeutic responses.
Provide direct nursing interventions including wound care, chronic disease management, respiratory support, and diabetes oversight.
Identify early signs of medical or psychiatric decline and respond appropriately.
Review and maintain vital logs, MARs, health notes, and other required documentation.
Ensure medication storage, disposal, and compliance with regulatory standards.
Uphold infection control standards and OSHA guidelines.
Maintain first aid supplies and ensure clinical readiness.
Participate in 24/7 on-call phone support (rotation).
Collaborate with ICM and CTI teams to support medical and behavioral stability.
Provide health education to individuals, caregivers, and staff.
Facilitate hospital discharge planning and community reintegration.
Coordinate follow-up visits with PCPs, specialists, and community providers.
Serve as a clinical liaison among families, program staff, and external professionals.
Participate in interdisciplinary case conferences and planning meetings.
Train and mentor DSPs on health tasks, documentation, and emergency procedures.
Maintain timely and compliant documentation per OPWDD/OMH/DOH standards.
