The Hospice Admissions RN conducts pre-admission evaluations and initiates hospice services by educating patients and families on available care options. They provide nursing assessments and develop initial care plans to optimize patient comfort and quality of life. This role requires a registered nursing license, CPR certification, and preferably hospice experience, with a focus on compassionate patient and family interactions.
Serenity Hospice is seeking a Hospice Admissions RN for our Portland Metro team.

Schedule: Monday-Friday, 8am to 5pm

What You’ll Do:
- Perform pre-admission evaluations for the appropriateness of services.
- Initiates the admission process by educating and informing patients/families of its services.
- Obtains informed consent
- Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
Qualifications:
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Must possess current CPR Certification
- 1 year of Hospice experience, preferred
- Must possess the ability to make independent decisions when circumstances warrant
- Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
- Must be knowledgeable of quality assessment and assurance procedures
