Minimum Requirements

  • Graduate of an accredited Physical Therapy program (DPT or equivalent).
  • Current, active, and unrestricted Physical Therapist license in Illinois prior to the start of the assignment.
  • Minimum of 1 year of clinical experience. Home health experience strongly preferred.
  • Knowledge of home health regulations, including Medicare guidelines and OASIS documentation standards.
  • Ability to perform comprehensive patient assessments and develop individualized care plans.
  • Strong clinical decision-making and critical-thinking skills.
  • Excellent communication skills with patients, families, and interdisciplinary teams.
  • Ability to work independently in a home-based setting and manage a flexible schedule.
  • Valid driver’s license, reliable transportation, and willingness to travel within the assigned territory. Mileage reimbursed between visits from first patient to last patient.
  • Current CPR/BLS certification (per American Heart Association or equivalent).


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Evenings.
  • Shift Details: Evenings 2000-0400.
  • Weekly Hours: 40 per week (weekly hour guarantee provided).
  • Days: Monday through Friday.
  • Coverage Area: Flossmoor office area; clinician must travel within assigned territory.
  • Primary responsibilities: comprehensive evaluations, develop and implement plan of care, deliver skilled PT interventions, complete OASIS and other compliant documentation, patient/caregiver education, interdisciplinary coordination, and monitoring patient progress.
