Minimum Requirements
- Graduate of an accredited Physical Therapy program (DPT or equivalent).
- Current, active, and unrestricted Physical Therapist license in Illinois prior to the start of the assignment.
- Minimum of 1 year of clinical experience. Home health experience strongly preferred.
- Knowledge of home health regulations, including Medicare guidelines and OASIS documentation standards.
- Ability to perform comprehensive patient assessments and develop individualized care plans.
- Strong clinical decision-making and critical-thinking skills.
- Excellent communication skills with patients, families, and interdisciplinary teams.
- Ability to work independently in a home-based setting and manage a flexible schedule.
- Valid driver’s license, reliable transportation, and willingness to travel within the assigned territory. Mileage reimbursed between visits from first patient to last patient.
- Current CPR/BLS certification (per American Heart Association or equivalent).
Assignment Details
- Facility Type: Home Health.
- Shift Name: Evenings.
- Shift Details: Evenings 2000-0400.
- Weekly Hours: 40 per week (weekly hour guarantee provided).
- Days: Monday through Friday.
- Coverage Area: Flossmoor office area; clinician must travel within assigned territory.
- Primary responsibilities: comprehensive evaluations, develop and implement plan of care, deliver skilled PT interventions, complete OASIS and other compliant documentation, patient/caregiver education, interdisciplinary coordination, and monitoring patient progress.
