RN Case Manager Job Details

• Location: Vicksburg, MS (Mississippi healthcare jobs)
• Employment Type: Travel Nursing Contract
• Duration: 13 weeks
• Start Date: ASAP
• Schedule: 8:00am – 4:30pm, 5 shifts/week (40 hours), rotating weekends
• Setting: Acute Care Hospital (on-site)
• Pay Rate: $25/hr
• Estimated Weekly Salary Range: $1810-$1940

Qualifications – RN Case Management

• Active Registered Nurse (RN) license
• BLS certification (Basic Life Support)
• Minimum 3 years of recent acute hospital case management experience (travel nursing experience preferred)
• Recent experience in discharge planning and utilization review in a hospital setting
• Permanent address must be at least 50 miles from facility
• Experience with Cerner EHR, MorCare documentation, and CarePort referral system preferred

Key Responsibilities

• Coordinate patient care and discharge planning to ensure safe and efficient transitions
• Conduct utilization review to optimize patient outcomes and resource management
• Document case management activities using MorCare and Cerner EHR systems
• Collaborate with interdisciplinary healthcare teams to facilitate referrals and care transitions
• Adapt to new referral systems as the facility transitions
