Requirements:

• Board Certified in Neonatology
• Minimum of 1 Year's Post Fellowship Experience in this role
• AZ Active License or IMLC Eligible for Compact License
• Minimum Level III Experience
• Ability to Offer Ongoing Availability
• Self Query NPDB
• Cannot accept providers who have not been clinically active for 1 year
• Providers must have at least 6 months of clinical experience within the specialty
• CV must be in MM/YYYY format with all gaps explained
