An Advanced Registered Nurse Practitioner (ARNP) is sought to provide primary and inpatient care management for elderly and frail patients in a hospital-affiliated outpatient program. The role involves evaluating physical complaints, educating patients and families, and collaborating with an interdisciplinary team while managing care across nursing homes and assisted living facilities. The Position: We're looking for an Advanced Registered Nurse Practitioner (ARNP). The pay range they are offering is $120,000 - $140,000 Salary.

Licensed Nurse Practitioner in the State of New JerseyMust possess a current NJ driver's licenseMinimum of 2 years working with the frail or elderly required Responsibilities:
Monday-Friday 8:00 A.M. - 4:30 P.M.On call 1 weekend per month (by phone only)Evaluate participant physical complaint and provide appropriate treatment.Educate participants and families as needed regarding self-care and specific medical conditions.Function as a member of the interdisciplinary team.Provide daily oversight of the PACE Clinic.Follow participants that are admitted to the hospital as needed.Perform rounds in the PACE contracted nursing homes and manage the complete care of participants in nursing homes, assisted living facilities and/or programs.Provide primary care responsibilities for inpatient care management.Regularly inform the interdisciplinary team of the medical condition of each participant, remaining alert to pertinent input from other team members, participant's and caregivers.Document care in the participant's medical record consistent with documentation policies.Please apply or send updated resume to or
