Registered Nurse Care Manager responsible for assessing, planning, coordinating and advocating for patient care from admission through discharge to ensure safe transitions and optimal length of stay. Works as part of an interdisciplinary team in an acute-care hospital setting, using care management and utilization management best practices and performance metrics. Requires RN license, BSN, BLS certification, and preferred 2–3 years acute care and case management experience.

Qualifications:
- Preferred Experience: 2-3 years acute care hospital setting, preferred
- Required Education: Graduate of an accredited School of Nursing, required
- License: Current RN License in the state of employment required
- Education: Bachelor's Degree of Science in Nursing, minimum required
- Certifications: BLS (Basic Life Support) at hire date, or within 90 days (about 3 months) of hire
