Minimum Requirements
- Board Certified/Board Eligible: Board Certified in ABEM OR ABFM OR ABIM (or osteopathic counterparts), must be residency trained in specialty
- Certifications: ACLS, ATLS, & PALS (If ABEM, need current ACLS and 1X proof of ATLS), CME past 2 years
- Licensure: Active State Medical License, CDS, & DEA (with IA address)
