Position OverviewWe are seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. In this role, you’ll assess patients, design personalized treatment plans, and provide hands-on therapy to enhance mobility, strength, and overall function for residents.
Key Responsibilities- Conduct patient evaluations to determine therapy needs and goals.
- Develop and implement individualized physical therapy treatment plans.
- Provide evidence-based therapeutic interventions to restore movement and relieve pain.
- Educate patients and caregivers on exercises, safety, and home programs.
- Collaborate with an interdisciplinary care team including nurses, physicians, and occupational therapists.
- Document patient progress and maintain accurate treatment records in Point Click EMR.
- Adjust care plans as needed to ensure optimal outcomes.
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited institution.
- Active Physical Therapy license in California (required).
- Minimum 1 year of experience, preferably in a SNF or rehabilitation setting.
- Excellent communication, interpersonal, and teamwork skills.

- Shift: 40 hours/week
