Physical Therapist (PT) – Skilled Nursing Facility | Riverdale, NY
MedAdventures is seeking a skilled Physical Therapist to join a skilled nursing facility in Riverdale, New York. In this role, you’ll help patients improve mobility, strength, and independence while collaborating with the interdisciplinary rehab team.

Your Responsibilities:
- Evaluate patients and develop individualized treatment plans
- Provide high-quality PT interventions to improve functional mobility
- Collaborate with PTAs, nurses, and other rehab staff
- Maintain accurate, timely, and compliant documentation
- Educate patients, families, and staff on mobility, safety, and discharge planning
Qualifications:
- Active New York Physical Therapist license
- SNF experience preferred
- Strong clinical reasoning, communication, and teamwork skills
- Commitment to delivering patient-centered, compassionate care
