Job Type: Full-time (On-site)

Responsibilities:

• Conduct patient consultations and follow-ups, perform comprehensive history and physical examinations, order diagnostic tests, review results, and report to the supervising physician as needed.

• Manage all aspects of outpatient patient care, including handling patient calls and collaborating with interdisciplinary team members.

• Maintain accurate and thorough medical documentation in the electronic health record (EHR) system.

• Educate patients and families on treatment plans, medication adherence, and lifestyle modifications.

• Participate in quality improvement initiatives to enhance patient care and outcomes.

Qualifications:

• Graduate of an accredited Nurse Practitioner (NP) approved program.
  

• Active and valid California NP license.

• National certification as an NP by a nationally accredited organization recognized by the state licensing board.

• Active DEA license.
