Monday Friday 8am -4:30 pm with weekend rotation of 1 /4 weekend requirement. Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. The RN case manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, promotes and maintains health of individuals through direct care, teaching, counseling and appropriate rehabilitative measures.
