• Provide skilled nursing care in home health settings ?

• Support patients to remain safely in their homes while promoting independence and wellness ?

• Collaborate with an interdisciplinary team to deliver comprehensive care

• Travel within your assigned territory, with federally reimbursed mileage ??

• Completion of an accredited LPN program

• Current LPN licensure or compact licensure recognized in Vermont

• 2+ years of home health, public health, or hospice experience preferred

• Previous home care experience is a plus, but not required – mentoring provided ?

• Ability to walk, stand, bend, lift, and move intermittently during shifts

• Lift, push, or pull up to 50 lbs safely

• Navigate stairs, maintain balance, kneel, crouch, twist, and reach

• Manual dexterity and visual/auditory acuity for patient care and equipment use ??
