Required Qualifications:
- Home Health experience - McKesson/Netsmart EMR experience preferred
- Collaborate with interdisciplinary team and colleagues to ensure a person’s needs are met.
- Provide occupational therapy care to a broad cross-section of clients living in central Vermont.
- Offer education and support to individuals to help them set and reach their goals of care.
- Support your clients’ desire to maintain their independence and to live in their own home.
License / Certification:- Vermont OT licensure
- General Certifications(ACLS,BLS/BCLS,PALS,NRP);
- Rehabilitation Certifications(ASHA,CHT,ACP Training )
Shift Notes: M-F w/ weekend rotation
