We are seeking a compassionate and experienced Adult Nurse Practitioner to join our team in providing primary care services to residents in assisted living facilities throughout New York.

• Provide comprehensive assessments, physical exams, and medication reviews

• Create individualized care plans in coordination with an interdisciplinary team

• Manage chronic and acute conditions

• Order and interpret labs and diagnostic tests

• Prescribe medications in accordance with NY State guidelines

• Educate patients and families on health maintenance & disease prevention

• Participate in chronic care management programs

• Maintain accurate documentation using eClinicalWorks (eCW)

• Master s degree in Nursing and completion of an accredited NP program

• Unrestricted NY Nurse Practitioner license

• Valid DEA license

• Eligible for Medicare/Medicaid reimbursement

• 3+ years of clinical experience (3,600 hours), ideally in long-term care or geriatrics

Locations: Multiple sites across New York State on-site presence required.
