Job Title: Family Medicine Physician

Job Responsibilities:
- Provide high-quality patient assessments, evaluations, and treatments within a patient-centered care model.
- Collaboration with supervising physician for ongoing patient management and care plans.
- Perform complete history and physical assessments as per patient necessity.
- Utilize basic laboratory services and perform necessary procedures.
- Engage in patient and family education to promote understanding of medical conditions and treatments.
- Participate in quality assurance and compliance audits.
- Maintain professionalism in alignment with the organization s mission and core values.
- Demonstrate flexibility in scheduling and coverage as needed.
- Contribute to clinical teaching opportunities with Family Medicine residents.
Qualifications:
- Board Certified or Board Eligible in Family Medicine.
- Experience in providing care to patients of all ages, particularly pediatrics to geriatrics.
- Proficiency in patient care management and documentation standards.
- Active member or maintain membership status on medical staff of local health systems.
- Experience or familiarity with EPIC EMR is preferred.
License and Certification Requirements:
- Must hold an active Indiana medical license or be willing to obtain.
- Current certifications in BLS and ACLS (or willingness to obtain).
- DEA certification (or the ability to obtain).
- Mandatory COVID-19 vaccination proof.
