PRN Nurse Practitioner Opportunity Near Yorktown, IN Are you a skilled Nurse Practitioner with a background in Family Medicine or Geriatric Medicine? We are seeking a dedicated Nurse Practitioner to join our team in Indiana on a PRN basis. This role is ideal for someone with experience in nursing home care, family medicine, or geriatric care who thrives in a fast-paced, autonomous, and self-managed environment. Position Overview: Job Title: Nurse Practitioner - Family Medicine or Geriatric Medicine (PRN) Shift: Day Work From Home: No Schedule: PRN (Part-Time) What You Can Expect: Practice Hours: Monday Friday, 8:00 a.m. 5:00 p.m. Enjoy a traditional PRN schedule that provides flexibility. Patient Volume: Average outpatient volume of 4-6 patients per day, allowing for thorough and personalized patient care. Call Responsibilities: Call responsibilities are based on provider staffing, with a minimum rotation of 1:3. Daily Duties: Care Planning: Collaborate with the PACE program s interdisciplinary team to develop comprehensive care plans. Patient Care: Provide day-to-day care for patients, manage and round on PACE participants admitted to long-term care facilities or acute care in participant facilities. Medical Assessments: Review medical histories and assessments, implementing care plans in collaboration with patients and their families. Outreach: Potential outreach activities with our Lafayette location to further extend care and services. On-Site Training: Training Requirement: On-site training is required in Indianapolis for up to 6 weeks, ensuring comprehensive preparation for the role. Qualifications: Educational Requirements: Master's Degree: Required Nurse Practitioner (NP) or Nationally Recognized Certification: Required Certification: Basic Life Support (BLS) Certification: Required (American Heart Association) Ideal Candidate: The ideal candidate will possess exceptional interpersonal skills and a strong desire to develop lasting relationships with patients and the community.
