Responsibilities- Home health setting providing services to patients in their place of residence
- Day shift from 08:00 to 16:30
- Assignment length is 13 weeks
- Uses Epic for electronic charting
- Responsible for evaluating, planning, and providing physical therapy services according to individualized patient care plans
- Reports to the VNA Clinical Team Manager, Rehabilitation
- Must operate within established organizational and departmental policies and procedures
- Team structure includes collaboration with other home health professionals
- Flu vaccination is required without religious or medical exemptions
- Parking may incur daily costs; public transportation options available
- Local candidates must have permanent residence at least 60 miles from Boston; local pay differential applies
- No pending licenses accepted for this assignment
- Professional attire appropriate for home health required
- Bachelor’s degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association; Master’s or Doctorate preferred
- Current and active Physical Therapist licensure in Massachusetts required; MA conditional licenses not accepted
- Current American Heart Association BLS certification required
- Minimum one year of recent experience as a Physical Therapist in a healthcare facility or equivalent, with home care experience required
- Current valid driver’s license, good driving record, and reliable transportation required
- Demonstrated knowledge in the principles of physical therapy
- Strong verbal and written communication skills
- Experience with Epic EMR preferred
- License required if applicable
