A successful candidate will have completed a Hospice and Palliative Fellowship program, and be board-certified through ABHPM. About the Program:
Providence Connections is a palliative care consultation service that has a team of experts who help patients and their families find needed resources to control symptoms associated with their illness while providing compassionate support. The team of physicians, nurse practitioners, palliative care nurses, clinical social workers and chaplains work collaboratively around high-risk heart failure and other conditions with Providence Heart Institute, as well as with Providence Cancer Center.
