American Traveler is seeking a Physical Therapist for a Home Health position requiring OASIS and HomeCare HomeBase (HCHB) experience in the Sonoma, CA area.
Responsibilities- Home health setting serving patients in Sonoma, CA and surrounding areas
- Visits are one-on-one with up to 6 patients per day, maintaining a standard of 30 productivity points
- Day shift with 8-hour shifts from 8:00 to 16:30, five days a week
- One weekend shift required per contract
- No floating to other units required
- No on-call requirement during this assignment
- EMR used is HomeCare HomeBase (HCHB); proficiency required
- OASIS documentation required for Start of Care visits
- Mileage reimbursement offered at IRS standard rates
- No standard attire or scrub color required
- Main responsibilities include providing skilled physical therapy services to home health patients and completing comprehensive OASIS assessments
- System groups clients to minimize drive time, but some travel may extend beyond a 50-mile radius
- Documentation and timely communication are essential responsibilities
- Free parking is available for staff
- Travel pairs are accepted but each traveler must have their own transportation and will be assigned to separate territories
- Former permanent employees must have a minimum 6-month separation and receive HR approval before returning as a traveler
- RTO (Requested Time Off) must be submitted at the time of application
- Assignments are open to local candidates within a 50-mile radius
- Active PT license required; license in hand required for consideration
- Minimum 1 year of recent, relevant skilled home health PT experience required
- OASIS experience required for Start of Care documentation
- Proficiency in HomeCare HomeBase (HCHB) required
- Current BCLS certification required
- Valid driver’s license and personal transportation required
- Not open to first-time travelers
- Pending licenses accepted if applicable
- Completion of a Hospice/Home Health checklist required
- Must provide two recent professional references from supervisors
- Work history from the past seven years with all gaps explained required
