Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Kentucky PT license required to start the assignment.
  • Current BLS (Basic Life Support) certification from AHA or ARC may be required.
  • Experience working in Home Health settings is required. Must be comfortable providing in-home care and documenting visits per agency standards.
  • Ability to travel within assigned coverage areas, including Todd County and Logan County.
  • Reliable transportation and valid driver's license required for patient travel.
  • Preferred: prior experience with electronic medical records used in home health and familiarity with home safety assessments.
