Travel RN – Case Manager
Location: Mission Viejo, CA
Pay:$3,219/week
Shift: Days | 5x8s (8:00 AM – 4:30 PM)
Start Date: 02/02/2026
Contract Length: 13 weeks
Position OverviewProvidence Health and Services is seeking an experienced Travel RN Case Manager for an acute care hospital. The role focuses on care coordination, patient assessment, discharge planning, and interdisciplinary collaboration. This assignment requires prior travel experience and expertise in acute medical care management.
Schedule & Unit Details- Day shift, Monday–Friday
- Patient ratio: 1:25
- Weekend coverage: Every other weekend
- Floating required to Laguna facility as needed
- Interdisciplinary support available
- Adult and geriatric patient population
Requirements- Active CA RN license (pending accepted if active at start)
- Minimum 2 years acute medical Care Manager experience performing care coordination
- Must have travel experience (first-timers not accepted)
- Proficient in Epic documentation
- Knowledge of HMOs, IPAs, DME/SNF/ambulance arrangements, and care for patients with complex social/medical needs (homelessness, mental health, foster care, substance abuse, protective services)
Clinical Skills & Experience- Care coordination and patient assessment
- Discharge planning and arranging DME, SNF, and transport
- Participation in family meetings and treatment plan development
- Regulatory knowledge: CMS, Joint Commission, OSHA, HIPAA, Workers Compensation
- Acute care exposure: ED, ICU, MS, Surgical Services preferred
