Travel Registered Nurse (RN) – Home Health

HumanEdge Health is seeking a Travel Registered Nurse (RN) for a 13-week home health assignment in Poughkeepsie, New York. This role provides in-home nursing care to patients within the community and does not require on-call coverage.

Assignment Details

Location: Poughkeepsie, NY
Start Date: January 20, 2026
Duration: 13 weeks
Schedule: Day shift
Shift Times: 8:30 AM – 5:00 PM
Hours: 8.5-hour days
On-Call: None

Responsibilities

• Provide skilled nursing care to patients in their homes

• Conduct patient assessments, develop care plans, and coordinate services

• Administer medications, wound care, and patient education

• Collaborate with physicians and interdisciplinary care teams

• Accurately document care and outcomes in the electronic medical record

Requirements

• Active New York State RN license

• BLS certification

• Minimum of 1–2 years of home health or home care nursing experience

Preferred Qualifications

• Experience with HCHB (Homecare Homebase) EMR

• At least 1 year of acute care hospital experience
