Immunization Requirements: Immunizations required for credentialing include the flu vaccine, TB status/X-ray results, MMR titer or complete immunization series, Hep B titer or complete immunization series, Varicella zoster titer or complete immunization series, and Tdap immunization within the last 10 years.
