The Case Manager Registered Nurse coordinates complex discharge planning and care for medically complex patients in a non-bedside, patient-facing role within a collaborative healthcare team. This PRN position involves working primarily daytime shifts with opportunities for continued education and leadership support. The role requires an RN license, healthcare experience, and the ability to collaborate with multidisciplinary teams to ensure safe and effective patient transitions.
Schedule : PRN; Shifts may be available any day of the week between the hours of 8:00 AM and 5:00 PM; must be able to train during the week for several weeks
Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte; will be eligible to float to other hospitals in the region
This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.

What We're Looking For:- Education: Associates Degree in Nursing or BSN, required. 4 Year / Bachelors Degree, preferred.
- Experience: Two or more years healthcare experience, preferred. Case Management or related experience, preferred.
- Licensure/Certification: Current RN licensure in state of employment, required.
- Additional skills required: Weekend and holiday coverage.
- Additional skills preferred: Knowledge/Competence of Microsoft office products and keyboarding.
- Complex discharge planning
- Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
- Collaborate with MSW case managers for guidance with pscyhosocially complex cases
- Facilitate coordination of care
- Accept responsibility, authority, and accountability for management of care
- Serve as a resource for discharge planning and care coordination for medically complex cases
- Maintain current knowledge of available community resources/post acute care options
