Job Type: Full-Time / Part-Time
Shifts: Days (No nights or weekends)
Healthcare Setting: Assisted Living / Long-Term Care

Key Responsibilities:

• Conduct comprehensive health assessments, including physical exams and medication reviews
• Develop and manage individualized care plans in collaboration with an interdisciplinary team
• Manage chronic and acute conditions, providing appropriate care and referrals
• Order and interpret diagnostic tests (labs, imaging, etc.)
• Prescribe medications within scope and regulatory guidelines
• Participate in chronic care management service planning
• Educate residents and families on health maintenance and prevention
• Attend interdisciplinary meetings and contribute to quality improvement
• Maintain accurate EMR documentation

Qualifications:

• Master's degree in Nursing and completion of an accredited NP program
• Unrestricted New York Nurse Practitioner license
• DEA license to prescribe medications
• Eligible for Medicare and Medicaid provider status
• Minimum 3 years of clinical experience (preferably in geriatrics or long-term care)
• Experience with eClinicalWorks (eCW) preferred
