Registered Nurse (RN)
Overview: Bushwick Center is seeking a Registered Nurse (RN) to join our dedicated team at our Adult Day Care. This role is focused on delivering high-quality clinical assessments, care planning, and ongoing support for residents through the New York State Uniform Assessment System (UAS-NY).
As part of our interdisciplinary care team, the ideal candidate will be a compassionate, detail-oriented professional who is experienced in conducting in-person assessments, developing individualized care plans, and working closely with residents, families, and healthcare providers to promote resident well-being and regulatory compliance.
Duties:
Perform thorough in-person clinical evaluations for existing residents according to UAS standards, agency policies, and state laws.
Create individualized service plans and determine eligibility for ongoing participation through detailed UAS assessments.
Evaluate potential new residents by considering clinical, social, and environmental factors to establish appropriate levels of care ensuring safety and well-being.
Gather and analyze clinical information from multiple sources to support patient care decisions based on clinical guidelines and professional judgment.
Maintain clear and effective communication with residents, family members, healthcare providers, and other stakeholders to guarantee assessment accuracy.
Apply a strong understanding of the challenges faced by vulnerable populations with chronic or complex bio-psychosocial needs.
Ensure all assessments meet regulatory and organizational requirements.
Requirements:
Valid Registered Nurse (RN) license.
UAS-NY Certification required
Experience or familiarity with UAS-NY strongly preferred.
Excellent communication, organizational, and interpersonal abilities.
Self-motivated with the capability to work independently.
Proficient computer skills and fluent English communication.
Prior experience in MLTC or Managed Medicaid environments is a plus.
