Compensation
Salary range: $135,000 $155,000 annually ($64.90 - $74.52 hourly)
Full-time, Monday Friday, 9:00am 5:00pm
Fully remote
On-call by phone for 1 week every 3 4 months as needed (very light call volume, averages 0 2
calls per day, rare emergencies)
About the Role
We are seeking a compassionate, experienced Psychiatric Mental Health Nurse Practitioner to provide high-quality, virtual psychiatric care to older adults with serious mental illness, substance use disorders, and dementia. This role is ideal for clinicians who value whole-person care, collaboration, and thoughtful clinical decision-making in a virtual-first environment.
You will conduct psychiatric evaluations and follow-up visits, manage complex patients with psychiatric and medical comorbidities, and work closely with physicians, therapists, care managers, families, and caregivers to support continuity of care and reduce unnecessary hospitalizations.

Key Responsibilities
• Conduct initial psychiatric evaluations and ongoing medication management visits
• Provide compassionate, patient-centered care tailored to older adults with complex psychiatric, cognitive, and medical needs
• Diagnose and treat serious mental illness, substance use disorders, and dementia-related behavioral symptoms
• Apply geriatric psychopharmacology principles and adjust treatment plans appropriately
• Manage crisis situations and determine when higher levels of care are needed
• Collaborate closely with physicians through regular weekly supervision and case review
• Coordinate care with primary care providers and make referrals to psychotherapy and care management as appropriate
• Engage families and caregivers in treatment planning when clinically indicated
• Maintain timely, accurate documentation and navigate the electronic medical record efficiently
• Participate in team huddles, case discussions, audits, and quality improvement initiatives
• Utilize virtual care technology confidently and professionally
• Contribute to pilot programs and adapt to evolving workflows in a startup environment
Required Qualifications
• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Active NP license and DEA in Florida or Texas
• Willingness to obtain licensure in additional states as needed (including Indiana, Arizona, Illinois)
• Prior experience providing virtual care or telehealth services
• 3 5 years of NP experience in acute or complex psychiatric settings such as inpatient psychiatry, ER, urgent care, IOP/PHP, ACT teams, nursing homes, or outpatient mental health clinics
• Experience treating patients with serious mental illness, substance use disorders, and cognitive impairment
• Strong clinical judgment and ability to manage complex cases and crisis situations
• Excellent communication skills with patients, families, peers, and leadership
• High level of professionalism, reliability, and responsiveness
Preferred Qualifications
• Credentialed with national insurance providers
• Experience working with older adult populations
• Experience in managed care, collaborative care, or value-based care models
• Comfort managing patients with significant medical comorbidities and social determinants of health
• Bilingual in Spanish and/or Haitian Creole (highly preferred; pay differential available)
