BAYADA Home Health has an exciting opportunity for a Full-Time Admissions Physical Therapist!! You will complete OASIS Starts of Care (SOCs) for geriatric patients living in Senior Living (Independent and Assisted Living) Communities in Newton, Wayland, Natick, Milford and surrounding towns. One year prior clinical experience as a licensed Physical Therapist is required and Home Health and OASIS experience is strongly preferred. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to patients, affording them the opportunity to receive the medical care required to remain at home.

Admissions (SOC) Physical Therapist Responsibilities Include:
- Complete OASIS Starts of Care (SOC) for geriatric patients in Senior Living (Independent and Assisted Living) Communities
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet
Qualifications for a Physical Therapist include:
- A current Massachusetts Physical Therapist License.
- A minimum of one year of recent work experience as a Physical Therapist
