The Licensed Behavioral Health Professional provides brief, evidence-based behavioral health services to support patients with mental health, substance use, and chronic conditions within a primary care setting. Responsibilities include assessments, therapeutic interventions, crisis management, care coordination, and patient education delivered in-person, via telehealth, or phone. This role collaborates closely with primary care providers to ensure comprehensive and coordinated care while maintaining appropriate documentation and billing.
Key Responsibilities:
Conduct initial and ongoing assessments, including diagnostic evaluation, risk assessment, and level-of-care determination.
Provide brief therapeutic interventions, crisis management, and patient/family education.
Maintain a minimum of 4 patient contacts per day (after 6 months).
Collaborate with primary care providers to develop and implement care plans.
Monitor patient progress, using standardized screening tools, and communicate updates to the care team.
Complete professional billing accurately and timely for behavioral health services.
Assess and manage suicidal, homicidal, or abuse risk; coordinate with crisis teams or authorities as needed.
Support population health outreach and proactive patient engagement.
Provide consultation and training to primary care staff on behavioral health and substance use topics.
Qualifications:
Education: Master’s degree in Social Work or Counseling.
Experience: 1–3 years clinical experience, preferably in primary care.
Licensure/Certification: Montana LCSW or LCPC license required; BLS required; credentialing with payors within 3 months of hire.
