Job Description:
The Physical Therapist in Home Health is responsible for providing physical therapy services to patients in their homes. The role includes conducting start of care, recerts, and resumption of care with a point system of 6.5 points per day. Regular visits include 5-6 per day unless additional assessments or admissions are required.
Requirements:
- BLS Certification
- ND Physical Therapy License
- Current Driver's License & Auto Insurance
- Home Health experience and OASIS training preferred
