Advanced practice provider (NP or PA) to work onsite in skilled nursing/long-term care facilities providing comprehensive internal medicine/family medicine care focused on wound identification, management, prevention, and interdisciplinary care coordination. Responsibilities include patient assessment, development and implementation of evidence-based wound care plans, documentation in EMR, participation in QAPI and regulatory compliance, and staff education/competency training. Position requires active NP or PA license, SNF/LTC or geriatric experience preferred (wound-care training provided), flexible scheduling, benefits, and clinical autonomy.
We are looking for an advanced practice provider (NP or PA) with experience in Internal Medicine or Family Medicine that is interested in working in the wound care arena. Prior wound care experience is not required - we will train you!
Working onsite in the Skilled-Nursing Facility (SNF) environment, this provider performs comprehensive patient evaluations, develops individualized care plans, and collaborates closely with nursing staff, therapy teams, and attending providers to optimize resident outcomes.
- Perform routine and targeted patient assessments for facility residents.
- Utilize validated tools to identify residents with wounds (or at elevated risk).
- Develop and implement evidence-based care plans.
- Create individualized care plans aligned with facility protocols and CMS guidelines.
- Collaborate with nursing staff to ensure proper implementation of appropriate interventions.
- Coordinate interdisciplinary care with facility's clinical team.
- Complete accurate, timely documentation in our EMR.
- Participate in QAPI initiatives, wound rounds, and prevention-focused quality efforts, when required.
- Ensure compliance with CMS regulatory standards.
- Provide bedside and formal staff education on wound care and prevention best practices.
- Support orientation and competency training for nursing staff related to wound care.
- Active NP or PA license.
- Experience in SNF/LTC, geriatric care, or wound care preferred, but not required.
- Wound care certification (WCC, CWCN, etc.) preferred, but not required.
- Excellent communication, teamwork, and documentation skills.
- On-site role within Skilled-Nursing or Long-Term Care facilities.
