Opportunity to coordinate care for complex/chronic patients in a stable day-shift clinic (Mon–Fri, 8–5). LPN Care Coordinator supporting physicians and clinical staff in ambulatory/care management at BHFC-White Hall. Day shift, Monday–Friday 8am–5pm; 80 hours bi‑weekly; location: White Hall, AR; ongoing/regular staff role. - Current LPN license in good standing (AR or multi‑state covering AR)
- CPR certification
- High school diploma or GED (if working in lab)
- Valid driver's license, insured reliable transportation
- 3–4 years' nursing/ambulatory or care management experience preferred

Shift: Day
Working Hours: Monday to Friday, 8 am to 5 pm
Summary: To work collaboratively with physicians and clinical staff to support patients with chronic conditions and/or complex needs according to guidelines established
Other information: • Current LPN license in good standing (either AR license or multi-state license to include AR), required • CPR certification required • High School diploma or GED equivalent if working in the lab • Three to four years' experience preferred • Ambulatory and/or care management experience preferred • Current Driver's License in good standing and reliable and insured transportation
This job will be authorized 80.00 hours bi-weekly.
