A Federally Qualified Health Center in Brawley, CA is seeking a Board Certified or Board Eligible Family Medicine Physician to provide outpatient medical care to underserved patients. The role involves diagnosing, treating, and managing patients with chronic conditions, coordinating care, and maintaining thorough documentation in a patient-centered medical home environment. - $270,000 - $370,000 / year

Expectations for new Provider: Family Medicine
- Board Certified/Board Eligible
- Ideal candidate will have strong work ethic, outstanding clinical skills and strong desire to serve the medically underserved
- 18-22 patient/day
- Monday Friday 8 hour shift. No weekends and no on call
- We currently have two midlevel FM we also have Peds, WH, BH that work at least 1 to 2 days per week.
- Ideal candidate will have a strong work ethic, outstanding clinical skills and strong desire to serve the medically underserved patient population
- Provides highest standard of outpatient medical care to patients. Including examination, diagnosis, treatment and rehabilitation.
- Responsibility for all his activities. Including accurate charting, good documentation of history, physical findings and assessment and treatment plans; and keeps track of all seriously ill patients this physician sees.
- Responsible for proper case management for patients with chronic conditions.
- Arranges timely consultations with specialists when needed by the patients.
- Knowledge and implementation of community-based, patient-centered medical home (PCMH) process with appropriate coordination of care.
