Exact dates and shifts TBD in March Shifts are randomly assigned by a scheduling tool and cannot be requested Work Details: Number of ORs: 11 in the main OR, 2 in the ambulatory center Patients per day: 50-60 cases per day Department description: Description: comprised of ortho, ortho trauma, general, general trauma, spine, neuro, cath lab, ambulatory and endo. EMR: Cerner and SaAnesthesia Hospital bed size: 317 Case/Procedure Experience: must have recent hospital experience. Interview Process: Dr. Weiss will interview candidates by phone. Practice Setting: Hospital Requirements: Board Certification: CRNA License required: AZ License, time to license DEA: No CSR: No Certifications required: BLS or ACLS Minimum experience required: Credentialing Timeframe: 30 days OT after scheduled shift - No weekly OT or after 40 hours NOTES: Licensing: If an AZ license is not active at the time of an offer we will need to provider the hospital proof that it has been applied for before they will release the application. Cerner training/orientation: This is required before starting. It is scheduled the week prior to the provider starting, assuming everything is going well with credentialing. It lasts approx. 4 hours and is remote. Compensation for this is the provider s hourly rate or $215hr, whichever is lower. If additional orientation is needed in person it will be done the day of or day before. Malpractice and disciplinary background issues: -If your provider has more than 500K in malpractice or more then 2 cases please reach out to me first with those details so I can see if they will be considered. -Recent case logs are required, if you provider does not have consistent logs within the last 12 months or has not worked in the last 3 months please reach out to me with details so I can check to see if they will be considered. Travel: Unless otherwise noted in agreements the following will be enforced (receipts must be provided) Flight: Economy Airfare only; first class and business class will not be reimbursed. Any airfare over $1,000 roundtrip needs prior NAH approval. Lodging: Not to exceed $200 per night (includes hotels and registered lodging through Airbnb/Vrbo) If booking direct outside of Airbnb/Vrbo the maximum reimbursable amount is the same ($200/night) and must have verified receipts (canceled check and statement). Shift gaps over 3 days are not billable unless approved in advance by Service Line Administrator. Rental car/gas: Standard/Economy rental car is reimbursable. Maximum daily rate is $100 (economy). Reasonable gas expense incurred to and from work and airport will be reimbursed. Shift gaps over 3 days are not billable unless approved in advance by Service Line Administrator. OR Mileage to get from home to facility, local mileage is excluded Hotel parking Baggage Fees: Standard baggage fees are reimbursable. Extra fees for checking golf clubs, snowboards, etc. are not. Airport parking: (economy only) is reimbursed or ride share services to get to/from airport are reimbursed; black car service is not.
