About the Role
As an APP on our team, you ll serve as the clinical lead for your patients delivering in-home primary, urgent, and palliative care. You ll work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full-time role with a schedule of Monday through Friday, no weekend work required.
Key Responsibilities
Deliver primary and palliative care to elderly patients in their homes (3 6 patients per day).
Perform physical assessments, develop care plans, and manage chronic and acute conditions.
Provide education and counseling to patients and families.
Manage medications, labs, imaging orders, and referrals as needed.
Participate in interdisciplinary team meetings and care plan reviews.
Maintain meticulous documentation to meet care quality and compliance standards.
Experience
3+ years of progressive NP or APP clinical experience
Strong background in geriatrics, internal medicine, palliative care, or end-of-life care
Home-based care experience preferred but not required
