• Aneurysm repairs
• Abdominal
• Arterial bypass
• Peripheral
• Vascular Access Devices
• AV shunt

Endovascular

• Bypass Surgery
• Femoral Popliteal Bypass
• Aortofemoral Bypass
• Stenting
• Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions
  

Treatment of

• Vascular Malformations
• Nutcracker Syndromes
• Lymphedema
• Gangrene of the limbs
• Peripheral Artery Disease

SUBMISSION REQUIREMENTS

• Board Certified Vascular Surgeon - REQUIRED
• Fellowship Training - REQUIRED
• OH License - Preferred , willing to accept providers needing to obtain OH license ( Note status at time of name clear)
• BLS & ACLS Certified - REQUIRED
• COVID Vaccine - REQUIRED, noted at time of name clear

Other Info:
The provider will be covering 4 locations
Practice Setting: Hospital, Will not require clinic hours
Average Number of Cases Per Day: Rounding at 4 hospitals with an average of 6 patients at each hospital
Documentation System/EMR: EPIC
Trauma Level/Designation: 1
Number of Operating Rooms: Each hospital has multiple
Patient Age Demographic: Adult

Minimum Board Certification: BC
