Opportunity Overview: - Location: Grand Junction, CO
- 15 -20 patients per day
- Flexible schedule Monday to Friday 2 days a week
- Day call on coverage days only
- Post-acute care inpatient setting
Responsibilities:- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
- Develop and implement individualized care plans
- Ensure residents receive high quality medical care and oversight
Qualifications: - Board Certified internal medicine (IM) or family medicine (FM)
- Active Colorado license
- Active DEA
CompensationUncapped earning potential: our compensation is a productivity-based model (fee for service); expected compensation is estimated salary range of $108,000 to $130,000 annually with no cap on productivity income potential with full benefits.
