This position seeks a board-certified or board-eligible Family Medicine, Internal Medicine, or Geriatric physician to join a non-profit organization providing comprehensive care to keep patients in their homes and communities. The role involves delivering primary and specialty services, caregiver support, recreational and therapy services, and occasional patient assessments in home or nursing home settings.
