ScheduleFull-Time, Day/Evening shift rotations with Every Other Weekend (EOW) and holiday requirements.
Job DescriptionThe RN will provide care relevant to the age of the patients on the assigned unit. Responsibilities include assessing physical and psychosocial conditions, collaborating with the interdisciplinary team, and contributing to individualized care plans for residents, particularly in adult and geriatric populations.
Responsibilities- Collect ongoing data related to residents' conditions and communicate findings effectively.
- Develop culturally relevant care plans involving residents and their families.
- Evaluate resident outcomes and recommend care plan revisions as necessary.
- Participate in the mentoring and orientation of new staff.
- Ensure safety and economic considerations are included in care planning.
- Current New York LPN Licensure.
- BLS certification (AHA or RQI).
- At least 1 year of experience as an RN in a Long Term Care (LTC) facility.
