Physical Therapist role in post-acute care; full-time/part-time; locations vary (home health, inpatient rehab, hospice); ongoing/permanent position. - Licensed PT in state of practice - DPT or PT degree - Experience in post-acute, home health or inpatient rehab preferred - CPR/BLS certification - Valid driver’s license for home health visits (if applicable)
