Location: Greenbrae, California
Schedule: Full-Time, 8-Hour Day Shifts (40 hours per week)
Required Experience
2-5 years of clinical practice preferred.
At least 2 years of direct experience in the relevant specialty strongly desired.
Previous experience in a medical office or outpatient clinic setting preferred.
Position Summary
Under the supervision and in collaboration with the physician team, the Advanced Practice Provider (APP) delivers high-quality healthcare through comprehensive patient assessment, diagnosis, and treatment. This role emphasizes health promotion, chronic disease management, and preventive care. The APP will perform in-office procedures, manage patient care plans, and serve as a trusted partner in optimizing patient outcomes within an ambulatory clinic setting.
Key Responsibilities
Clinical Care and Assessment
Conduct thorough patient histories and perform physical examinations.
Order and interpret diagnostic and laboratory tests.
Identify and evaluate risk factors, chronic conditions, and social determinants of health.
Formulate accurate diagnoses and develop individualized care plans.
Treatment and Implementation
Prescribe or dispense medications in accordance with state and federal guidelines.
Educate patients and families on treatment options, medication usage, and preventive health strategies.
Provide follow-up care, telephone triage, and ongoing monitoring of patient progress.
Collaborate closely with physicians and interdisciplinary teams for continuity of care.
Utilize Epic EHR for comprehensive patient documentation.
Qualifications
Education & Certification (Physician Assistant or Nurse Practitioner)
Graduation from an accredited Physician Assistant or Nurse Practitioner program.
Current and unrestricted license to practice as a Physician Assistant in the State of California.
Board Certification
Active DEA license transferable to California.
BLS and ACLS certifications (American Heart Association).
Eligible for Medicare and Medicaid enrollment.
