- Start Date: ASAP (Oct 15 preferred)
- Assignment Length: Through end of year with opportunity to extend
- Schedule: 40hrs guaranteed Mon-Fri 5x8s or 4x10s
- Setting: In-home visits to health plan members (primarily 65+)
- Daily Volume: 5-6 per day
- Independence: Visits conducted solo; Check-In Specialist handles member check-in
- Training/support: Clinical Trainers + Practice Coordinators assigned; VP of Clinical Content & Services (NP) involved
- Primary Location: Kent County. Providers may be reassigned once location is cleared
- Rate: $90/hr, all expenses paid (hotel, car, flight if needed)
